can someone throw more light on the performa, I have not received any email or post mail from FMS on the performa and I am not able to find it on the website either.
@Vibhu0305 performa means your form and documents. if you hae already sent them you dont need to anything else.

@Supersub Please carry all your marksheets(Degree,10,12),10 & 12 Certificate,Cat Score Card, Caste certificate if any, work ex certificate or last 3 months payslips if any..
@Supersub ...u need to bring all the certificates of all your academics,starting from 10th to till the last academic exam u have appeared for..then u need to carry your competitive exams marksheet,like cat,xat or mat whatever it may b..and work experience certificate if any..

I had sent my form 2 weeks back...that time the form didn't mention anything about Date of Birth Certificate. Now after getting the fms gdpi email i downloaded the same form once again from fms website and now it says that we need to send DOB certificate also. Please suggest.
Did amybody receive any email from FMS in response to the request of date change? I have sent the mail 3days back but yet to receive any response. seniors kindly help. we need to book flight tickets, if the response comes too late, flight prices will sky rocket.

According to seniors, if your marksheet had your DOB mentioned, you don't have to send anything. Else, just send the DOB proof (certi or 10 or 12 marksheet) to the address mentioned in proforma. If even that's not possible, just bring the same to the GDPI process

1) Do we need to bring copies of our marsheets/certis? Do they need to be attested?
2) I hav my slot on 22nd march @ 11.00am. Would 1 hr be enough to reach the campus from airport by cab?
