How to withdrawal admission

1. I have clicked on the "Withdrawal admission" button on the admission page.

2. On the accecptance email it was written that a written application should be sent to chaiman of admission, does this mean i need to send a hard copy of application, and to which address?, OR if i have to e-mail the application, then to which email address,
