1. My name in the matriculation cert is "M S V DURGA PRASAD" and in attestation form it is asked to fill full name....so do i need to expand that "m s v" also or fill it as in matric cert?
2.Coloumn no.5 mein....apna details dekhar family member ke naam dena hai or "all the details like nationality place of birth and all"of family members also?
If it is for all the family members, the place is too less..... how to fill?
3.in coloumn 10, it says "educational qualification....since 15th years of age", does that mean after 15 years of age or for the last 15 years?
4. In point no. 13, can the reference be anyone near our house, or should be someone who holds a good position...i mean governemt position or college ke faculty etc.,
and should only the names be mentioned or their address, phone no. also?
aur yeh reference kyun puchthe hain?
5. Most important doubt....in attestation form...the 6th page is Identity Cert...so attestation by gazetted officer should be there in the 6th page alone or all the 6 pages of the attestation form?

1. better jo tumne ssc k liye bhar wahi bhar do
2. likhna to whole family ke liye hai but jis tarike se likha hai apna hi aa jaye to bahut hai. write your only, if space permits write ur father's too.
3. from 15th year generally 10td and above
4. anyone near in your locality, tym pass ke liye reference hain
5.only last page, itna vella nahi hota gazetted officer
im also 4m WR.....even i got jus 23rd nov ......der is no oder date.....andheri...

Point 12 on Attestation form! No to be striked for "NO" or Yes to be striked. I have never seen more ambiguous language used in a form ever in my life!

Specific answers to each of the questions should be given by striking out “Yes” or “No” as the case may be.

bhai fir to YES ko cross kardo, ya dark kar do pen se, so that only NO is visible,
