I have a few doubts in filling antecedent form. Plz help me...

1. (sl no.1) 'full name'. No need to expand my name. right?

2. (sl no.9) 'age as on date'. Can it be the date on which i fill the form?

3. what to right under 10th std subject? 'languages & science' is it ok?
