Do I need to submit soft copy of NOC before 29 July also? As it would be next to impossible for me to get that.

1--is it mandatory to get the biodata form attested.
2--do we need to submit anything else apart from biodata form and identity certificate.
3--what do we need to put in testimonial column
4--under point 23.nationality f father and mother needs to be given or fathers nationality and mothers name
5--is work experience mandatory to be disclosed as i have misplaced my payslips.
