Should I upload original category certificate or self attested will be sufficient just like with CAT form?

Hey. I worked for a company for 5 motnsh last year and did not get work ex certificate as they don't provide one before 1 year. I have joining letter and pay slips. This year I am working in another company for 11 months and I have pay slips and joining letter. What should I upload as work experience certificate? Also should I mention about my last company or not?

1. If we are currently working in our organization, what should be mentioned in the "to date" of the work experience section?
2. Is it okay if we upload any one of the documents as proof for work experience?
3. Are there any other documents that need to be uploaded apart from work experience document?
