since it is photocopy- so only attestation is needed... if its original then no need--- attestation is to add credibility to certificates-xerox
to be on safer side-- i think it's better to get attestation on certificates also?

I think you need to write 2 attempts in case you've failed in your final semister's exam.

I think u should write like this in the attempts column: 2(so and so paper from so and so yr(1st yr or 2nd yr)). they'll however notice urs is a 5 yr term when they see the date of passing graduate college. so jst make clear which paper from which yr u had to attempt again.
