Do we need to attach self attested copies of certificates to all 5 sets? Or to only the original set?

Today I got sign from Branch Manager of SBI for my Character and Identity Certificate, but the manager used blue nd red colour pen to sign it. When i Asked to sign in Green, he said that he should sign only in blue along with the SBI seal, whether it is mandatory to get sign in Green ink? anyguys clear my doubts pls...

1) My cousin works in a nationalised bank. Can I put his name as one of the referees?
2) The gazzeted officer who attested my documents, can she be put as one of the referees in the form ?
P.S.- Gazzeted officer is my aunt and her son is my cousin

1.For two respectable degrees, can we put any friends or ex colleague's details? Could the gazetted officer, who has signed in identity certificate, also sign the character certificate and be a refree too? 2.For co-curricular activities, do we need to mention district, state or national level accomplishments only or school and college level also acceptable?
