dont send a hardcopy of ur signature/LTI..!! put ur signature and thumb impression ON the printed form and send that form... 
Puys, I am facing a strange situation..
When I made the payment and I printed the acknowledgement receipt, the form I got had my photo but the details were of someone else...I was surprised but when I logged in using the credentials I got as sms and I printed the ack form, the details were mine but not the photo.
What is this dilemma??? How could such a thing happen???
What should I do now??
Also in the details I have forgotten to write the city name... 
Should I write to the FMS admissions department?

very unusual indeed. call the helpline and mail them ASAP.!!

I know I shouldn't probably say it, but this is so funny man.. lol..

And yeah, u should definitely call them for this! As far as I could gather, they are cooperative in nature soo you should get the problem resolved. Good luck

I wanted to ask, if you have any idea about entry for NRI's, can they apply through their GMAT scores online??
Really confused on how to apply???...can't come n give CAT, as i'm working outside the country. I tried calling FMS but never connected through to Mrs/Miss Poonam, as she handles NRI admission, she never picked up my call and no1 else knew anything about it. If NRI's can apply through GMAT, what is the min score for application, it would be very helpful if you can answer my query as today is the last day for registration, I had mentioned my query earlier but no1 answered it.

And Since you have the details of the other person also, Inform him (or her, if you are Lucky! ) too please. That person's form must have the same problem and he/she must also be wondering for the same reason.
And don't worry abt it, its not ur fault, you will get it corrected by them only. These kind of things happen sometimes.

Guys, more abnormaility to follow...
When I tried to login again, again I saw different details(this time a different person) and my photo...
I tried even another time and bang on the details and photo both were mine..
Any idea How is this happening??

sounds more of a databse issue from FMS side. happens while testing(should not be the case when the reg have already started) . well no point thinking how the issue came up at the first place. if now you can see your details and photo then go ahead and take the print .

It seems like the DB issue but here is more to come...Now everytime I login and try to print details, I am getting forms of different persons...Possible information securuty breach...Really how could such a thing happen to FMS??? I would have posted all the pdf's here but then it would be a security breach from my side...
