@mayurdhingra Can we submit the form without uploading the LTI/ thumb impression. for me its asking to upload it even it tried for again and again. what to do? i dont have soft copies of thumb impression plz help.
Hi, Today 14/12/2012 is the last day..can you tell me till what time can I fill the form? will the form be active till 12 in the night? Please reply ASAP...I can only fill it after I am back from office tomorrow at about 8...Please help me out ASAP

It happened with me too, I made a fool of the s/w by uploading blank images. U can do the same, when you print the form sign and put the thumb impression there.

Yeah. It should be open till 23:59 hrs... But I suggest you do it as soon as possible. There is a good chance of server problem in the last minute..
Thanks a lot...One more query, if I do upload the thumb impression, picture and signature and am not a category student, I hope I will not be required to post anything to FMS. Is that correct? Please help me out ASAP 😃

You will have to take a printout of the acknowledgement form and send it to the FMS office along with an extra passport size photo
puys how can we confirm if our forms have reached their office?
this time's reg procedure is just riddled with problems all over

The admin office accept couriers. So you could confirm it like that or you could send via register post in which there is an option of confirming delivery....

I have already registered and sent the acknowledgement form by Speed Post. It's been over 10 days now and i have still not received any kind of confirmation of receipt on my Home Page. I checked with the Speed Post Tracker and it showed that my post has already been delivered at FMS. When I tried to contact FMS through the Helpline Number mentioned, the guy on the other side was very rude and arrogant and hung up on me without even answering my query. So puys please clarify me in this regard, whether any confirmation of receipt will be shown on the home page?? Really pissed off by the tiring formalities and myriads of glitches!!
And about the photograph, i have pasted my photograph in the space provided. Is that all or do we have to send an extra photograph excluding the one already pasted? HELP!!

Hey HellboyXtreme! From the way you sound, you want to be 101% sure that you are perfect in the process. Anyways, since you have not attached the extra photo, I suggest you send the form again with an extra photo. This time, send via courier or register post so that you can a confirmation from the agency of mail. I don't think you will be getting any confirmation from the admin office.

1. Have read somewhere over here - till 31 March 2013.
2. If working - 'Yes'. Then mention your designation and Organisation Name.
3. Send the ACK form enclosing DD [if applicable], one passport size photo and cast certificate [if applicable].

1. If you are currently unemployed, fill 'No'.
2. Certificates need not be reproduced now but would be required during later stages.
