In my opinion, 6 months is decent enough time for a person to know things about you if you had worked closely on a day to day basis. so you can get references from your new project as well.

they will get to write a brief note about you and regarding your positives and negatives. You will be notified by email once your referees send their recommendations.

But now she is on Sabotical and will be moving abroad in a couple of months. Will it be ok to take recommendation from her?

As i mentioned earlier, in my opinion it is completely fine to get a reco. from her. Clearly state the nature of the realtionship and furnish correct contact information while providing details about the referees.

@facemash @abhi.iimb Pardon me if this has been answered, but could you please let know what all is asked in the questionnaire. i just convinced the Managing Director of my company, thinking its a regular 1 page affair. I hope he would be able to give a reasonably good 'reference' with the questionnaire:)
@AcU I am an aspirant just like you. So tagging me won't help much. But, I've heard that there are 1-2 questions on ur strengths and weakness to be answered in 1 line rest is rating based.
