Someone plz clarify point no 5 on the call letter. Do we only need id proof or an identity card as well because i dnt have my college id

In call letter it is mentioned that no column should be left blank. So does that mean we have to write "Not applicable" or something like that if the column doesn't apply to us? For example, criminal case if any, caste certificate, etc. Please reply soon.
