Purchase the form from the Meerut GPO (Head Post Office as psc up forms are generally available at the head post offices of the City)
No restrictions on no of attempts

Ank sabse pehle to tumko form praapt ho gaya iski badhayi
ok ab aate hain aapke dwara pucchey gaye prashnon parAns no 1. Pariksha code mein likhye > 1
Ans no 2. Pariksha ka Year mein Likhiye > 2012
Ans no 3. Column no 9 jo ki educational qualification se sambandhit hai usmein sabse pehle 9(i) Mein Yes k sammukh(Samne) Oval ko bharein
Column 9 (ii) mein jis subject se aapne Degree exam pass kiya hai uska code bharein jese aapne commerce( Vanijya) se degree pass ki hai to Code 06 bharein( bakki column ko khali chhor dein).
iske baad agar aap PG pariksha pass hain to column 9(iii) ko bharein anyataha khali chhor dein,
Column 9(iv) aap par tab lagoo hota hai yadi aapne PG pariksha pass ki hai,
Column 9(v) mein other essential qualification mein wale column ko jis jis subject k saath aapne degree pariskha pass ki hai uss prakar bharein
Column 9(vi) aur 9(vii) ko khali chhor dein

Correct! Not only in Hindi-bhashi States but also in other States. Like Maharashtra PSC has paper in Marathi that is a big deterrent for me to fill the form!! 😉
