You need to attach your NC certi as well.. Please get it first thing on mon mor n send them..

Yup.. Nothing more to do ol now..
All you have to do is complete the form and mail it to them..

Bro...Even I thought that I have missed writing city but when I mailed the admissions dept of FMS, they told me that I have written the city in the form but it won't be visible in the printed acknowledgement form. So relax, mail them , they will surely help you out :)
I was trying to pay via online mode. I have entered every required details & in the end it is showing Payment NOT successful.
Is there any workaround?
Now the payment is accepted. Probably server issue.

If you cannot copy-paste anything then type it down,will solve the issue and no I do not think the date is extended.

I will mail them regarding this....but for now I will be sending the form as it is tomorrow by speed post as the 21st deadline is getting very close.
