You ll need photocopies of appointment/offer letter along with Current month's pay slip.
because ur payslip only tell that u r currently working but since when, can only be told from ur offer or appointment letter.

Though, I would still suggest u to fill the form in handwriting.
And if u still want the word format of the form, u can download a free version of "pdf to word" converter from net and convert it urself.

No, you are missing nothing. Post it, but do remember not to staple the application form and use gem clips. Also do not fold the application. Send it in A4 or larger sized cover.

You can come here in the IIT premises and drop it in the Dept office. You can drive towards the Gajendra circle, a famous landmark here in IIT. DoMS is located near it. But do remember to follow the instructions, as mentioned in the application form.

I have got a Rank of 864 in JMET 2008 and I am applying to DOMS.
I had a query regarding the application form.
Do I have to submit the work-exp letter by my current employer duly signed by HR ?
It will be difficult getting it as this point of time as i will have to tell the reason before getting the letter from HR.
Is it OK if I send a copy of my offer letter as well as the current salary slip.
Please reply ASAP.

Ideally the letter from your HR stating the time period of your employment would be the best. However, we understand that you can't ask for such a favour. So send in your appointment letter and your latest pay slip. That should tell us from when you have been employed and that currently you are still with that company.

I have a Transcript provided by my college which contains the course details, grades and CGPA for each semester. I have got it attested and plan to attach with the form. Will it suffice for the Academic certificates as well as the Detail marksheet. I am asking this coz our college also gives detailed semester level marksheets (for which I don't want to hunt for attestation coz the deadline is approaching)..

3. Word limit of SOP.
4. Number of freshers in your batch.

Is it fine if I submit the pay slip as it is or do I need to get this authorised?????

Detailed semester level marksheets would be required. Please get them attested and send them with the application form. I would suggest so because the faculty would want to know the specifics of your marks, course and subject details.

1. We are not sure about the exact numbers. Moreover, the dept office would term this information as privileged. However, the conversion ratio for each seat is around 1:8.
2. We have 62 students in the current batch.
3. The word limit for the SOP is 500 word. This information is already present in the form at our website.
4. We have 12 freshers in the 2007-09 batch.

Right now you can submit your appointment letter along with the latest pay slip. However, you must also submit a note undertaking responsibility to bring along your work experience certificate when you appear for GT/PI, if you are shortlisted. The work experience certificate becomes important because the authorities at IIT Madras need to ensure that they are interviewing and letting in people with genuine credentials.

Yes, because that would help the committee of professors here know that you have been employed with X company since so and so date, till the present date. But as said above, you must also submit a note undertaking responsibility to bring along your work experience certificate when you appear for GT/PI, if you are shortlisted.
